Refined Contouring. Restored Balance. Surgical Precision

Overview
At The Plastic Surgery Group of San Antonio, we believe the neck is not merely a transition between the face and body—it is a defining element of facial harmony. Our practice offers advanced neck procedures for patients seeking elegant, sculpted, and lasting results.
With surgical precision, deep anatomical knowledge, and an artistic eye for balance, we customize each treatment to enhance the natural contours of the neck and jawline. Whether you're concerned about fullness, laxity, or aging changes, we offer sophisticated solutions few practices perform routinely.
Our Signature Neck Procedures
Deep Neck Lift
A highly specialized procedure that addresses the deep structures of the neck—removing subplatysmal fat, refining digastric muscles, and adjusting submandibular glands when needed. It is ideal for patients with hereditary neck fullness or those who’ve developed heaviness not addressed by traditional lifting. It is often combined with Deep plane facelift or chin enhancement for harmonious, high-definition results.
Lateral Skin-Platysmal Displacement Neck Lift (LSD Neck lift)
A preservation neck lift technique which involves transecting the platysma to allow a laterally-based hammock for treatment of platysma banding and excess anterior neck skin. This remarkable procedure was described by Dr Mario Pelle Ceravolo after intensive research on band recurrence and best practice techniques.
Direct Neck Lift (Subplatysmal Contouring)
Goes beyond surface fat to recontour the deep neck space. Through an isolated submental (or below chin) incision, the deep neck anatomy can be safely recontoured for a sleeker, more chiseled appearance. There are no incisions around the ears - everything is done through a small, well-hidden below chin incision. A powerful, minimalistic procedure with maximal results.
Chin Implant with Neck Refinement
A well-proportioned chin is key to neck and jawline definition. We may recommend a chin implant to restore balance and structure. Enhancing skeletal support improves the entire lower face and neck aesthetic.
Platysmaplasty (Muscle Tightening)
This procedure refines neck contour by tightening vertical bands of the platysma muscle. Possible types include anterior corset platysmaplasty or lateral muscle suspension, depending on your anatomy.
Liposuction
A workhorse. Sometimes all you need is a little chin lipo (chipo). The critical issue here is whether you are actually a good candidate or not. This is best determined after physical examination with an experienced clinican who can tell if you have excess submental fat (a good candidate) or if the fat is below the platysma (a bad candidate).
Energy-Based Neck Tightening
For patients not ready for surgery, we offer liposculpture with adjunctive radiofrequency or plasma energy (such as J Plasma or Renuvion) to firm the skin and redefine contours. This procedure is minimally invasive with no large incisions, minimal downtime, and natural results. Not everyone is a candidate, however. We are here to help guide your decision.
Deep Neck Lift: The Foundation of Elegant Facial Rejuvenation
A beautifully rejuvenated face is incomplete without a refined, structured neck. The deep neck lift addresses the underlying anatomy beneath the platysma—removing deep fat, refining bulky muscles, and contouring the submandibular area—to restore youthful definition and balance. Often paired with a deep plane facelift, this advanced technique creates a seamless transition from the face to the neck, enhancing the overall harmony and sophistication of the profile.

Deep Neck Lift with Contouring
The deep neck lift is a procedure designed to address the structural causes of neck aging and contour irregularities at a level not reached by traditional skin-only or superficial neck lifts. Unlike standard techniques that focus primarily on tightening or removing excess skin, a deep neck lift redefines the cervicomental angle by surgically addressing the deeper anatomical layers—including subplatysmal fat, the medial edges of the digastric muscles, and, when indicated, submandibular gland prominence.
This procedure is particularly beneficial for patients with congenital or age-related fullness under the chin, poorly defined jawlines, or those who have not achieved desired results with liposuction alone. By directly modifying the deep tissue architecture of the neck, the procedure yields sharper contour, more enduring definition, and a natural transition from the jawline to the upper chest.
Deep neck contouring requires both technical mastery and aesthetic sensitivity. With extensive experience in facial anatomy and a reputation for precision, Dr Hogan customizes each surgical plan to the patient’s unique structural anatomy. In appropriate cases, the deep neck lift is complemented by chin augmentation or deep plane facelift techniques to further enhance harmony across the lower face.
Because the procedure involves advanced anatomic dissection and a nuanced understanding of facial balance, it is not commonly performed in general cosmetic practice. At The Plastic Surgery Group of San Antonio, it is a cornerstone offering—delivered with the highest standards of surgical care, safety, and artistry.

Before & After
Is Neck Contouring Right for Me?
If you’re noticing a poorly defined jawline, neck fullness despite a healthy weight, visible muscle bands, or loose skin, you may be a candidate for advanced neck rejuvenation. Many patients also seek this procedure to restore harmony between the face and neck. During your consultation, Dr. Hogan will perform a detailed anatomical evaluation and recommend a personalized treatment plan—whether surgical, non-surgical, or combined—to help you achieve refined, natural-looking results.

What to Expect: Deep Neck Lift and Contouring
Your procedure will be performed in a private, AAAASF-accredited surgical suite—an environment that prioritizes patient safety, comfort, and discretion. Dr. Hogan uses advanced techniques under general anesthesia or deep IV sedation, depending on the complexity of the case. Surgery typically lasts 2 to 3 hours, and patients return home the same day with a light compression dressing and detailed aftercare instructions. Every patient receives Dr. Hogan’s personal cell phone number to ensure immediate, direct access throughout the recovery process.
The deep neck lift delivers powerful, long-lasting results by addressing the deep structural causes of neck heaviness—offering more than surface improvement. With precision, anatomic knowledge, and artistic vision, we ensure each outcome is not only technically successful but also naturally elegant. With direct surgeon access and a highly personalized approach, your experience at The Plastic Surgery Group of San Antonio is designed to make you feel confident, cared for, and completely supported at every step.
Recovery is generally well-tolerated. Most patients experience swelling, tightness, and a sensation of fullness rather than pain in the first few days. Some patients have hemostatic net sutures (Auersvald sutures) to help control post fluid collection and results. These sutures are usually removed by day three or four and the rest by day seven, and most individuals feel comfortable resuming light activities by the end of the second week. Red light therapy and lymphatic massage are included as important adjuncts in your recovery. Visible swelling continues to resolve over the first few weeks, with results becoming increasingly refined between weeks three and six. Full results—marked by a sculpted neckline and improved jawline definition—are typically visible by three to six months. We recommend wearing a supportive garment as directed and avoiding strenuous activity for three weeks.
Results: Sculpted, Natural, and Enduring
The results of a deep neck lift are often described as transformative yet remarkably natural. Rather than appearing overly tight or surgically altered, patients achieve a refined jawline, a more sculpted neck contour, and a smoother transition from chin to clavicle. Because the procedure addresses the deeper anatomic structures—rather than just skin—the results are both more comprehensive and longer-lasting than traditional or superficial techniques.
Incisions are carefully placed in discreet locations, typically at the ears and, when necessary, a small one hidden beneath the chin. Dr. Hogan takes great care to minimize visible scarring by using meticulous closure techniques and placing incisions along natural creases or shadowed areas. For most patients, scars fade significantly over time and become virtually imperceptible. With proper care, they are easily concealed even with short hairstyles or up-dos.
Swelling and tightness are expected in the first several days, with sutures typically removed within a week. Most patients return to light activity and social settings within two weeks. As swelling subsides, a more refined and defined neck contour emerges, with full results appreciated between three to six months. Because the structural foundation is addressed, results tend to age gracefully and are often maintained for many years without the need for revision. Dr. Hogan’s goal is always the same: a timeless, elegant result that enhances your features—not alters them.
